Hi, I'm Swapna, the founder of The Gradient Health.
For over 26 years, I've had the privilege of caring for patients of all ages from children to older adults in Primary care, Women’s health , Urgent care, Mental health and Correctional medicine. These experiences have taught me that true health is about more than treating symptoms; it's about understanding the whole person.
My passion for hormone health, metabolic wellness, and healthy aging grew from seeing how many people struggle with fatigue, weight changes, poor sleep, low energy, and hormonal imbalances without finding lasting answers. I created The Gradient Health to provide personalized, evidence-based care that helps patients feel heard, supported, and empowered on their journey to better health.
I'm also currently pursuing my Psychiatric Mental Health Nurse Practitioner certification at Florida State University, allowing me to further understand the connection between physical health, hormones, and mental well-being.
